New Client Orientation
Welcome and thank you for allowing us to serve you! Please complete each of the below sections (1-7) and email/fax any of the documents generated to: firstname.lastname@example.org or fax them to: 469-277-3186. Please let us know if sections do not apply.
1. Client Information (required)
Please complete this basic form so that we get an understanding of your Company and the types of transactions.
2. Banking/Credit Cards (required)
We request "View Only" access to your bank accounts and credit cards so that we can proactively record your transactions and reconcile your accounts timely. (Note: One form per account/credit card)
Please complete one form per account.
3. Payroll (if applicable)
Please provide us with a report as of the last quarter in excel or PDF. This report should be a payroll register that shows how much each employee made from the beginning of the current year to the last payroll.
You may need a second payroll register as of the last reporting quarter. For example, if you are starting payroll with our firm June 1, we will need two payroll registers.
4. Debt and Leases (if applicable)
It's a good idea to keep a list of all the existing debt and leases so that we can assist in correctly amortizing the payments. Please complete the "Debt and Lease Schedule" with any debts and leases.
5. Fixed Assets (if applicable)
Listing of Fixed Assets is necessary for us to identify the property and equipment owned by the Company and to help us determine the deprecation.
6. Taxes (IRS) (recommended)
An IRS power of attorney will allows us to obtain copies of notices that are sent to you. Additionally, we can call the IRS and represent you.
7. Construction Contractors Only
Construction Contractors have two sheets that are combined within this excel spreadsheet. 1. We need a schedule of completed jobs as of the last period ending date; a schedule of jobs in progress as of the same period ending date.
